Young Adult Coordinated Entry Toolkit Posted

Connecticut Coalition to End Homelessness

Young Adult Coordinated Entry Toolkit, a new resource developed by the Reaching Home Coordinated Access for Youth and Young Adults subgroup provides guidelines for linking young people to community services. 

The guide is a living document and includes service connection to the following state departments: Mental Health and Addiction Services, Children and Families, Education, Social Services, Developmental Services, Veterans, and other services such as employment and legal services. A feedback component is embedded within the toolkit for providers and community members. 

The Reaching Home Coordinated Access for Youth and Young Adult Subgroup is tasked with developing an effective crisis response system for youth and young adults experiencing homelessness in Connecticut. The group is staffed by the Partnership for Strong Communities and is co-chair by Katie Durand, CT Department of Housing and Mimi Haley, Connecticut Coalition to End Homelessness.
